• Please complete the Equine Session Form and the Equine Treatment Form prior to the first session. 
• The Veterinarian Referral Form is to be completed while horse is under strict care of a veterinarian for an injury or significant health concern.

• Please complete the Human Session Release Form and the Human Initial Questionnaire Form prior to the first session. 
• The Doctor Referral Form is to be completed while client is under strict care of a physician for an injury or significant health concern.
